Clinic Accreditation

The clinic accreditation standards are set through Minister of Health Decree (KMK) No. HK.01.07/MENKES/1983/2022: 3 chapters (Clinic Governance; Quality Improvement and Patient Safety; Delivery of Individual Health Services), 22 standards and 104 assessment elements.

Mandatory requirements

All of the following requirements must be met before a survey request can be verified.

  • A valid business licence and registration with the Ministry of Health
  • The technical officer in charge is a medical practitioner holding a practice licence (SIP) at that clinic
  • All care providers hold a valid professional registration certificate (STR) and practice licence (SIP)
  • The ASPAK facility & equipment system is 100% complete and validated by the District/City Health Office
  • Reporting of INM and IKP for the last 3 months (initial survey) or the last 12 months (re-accreditation)

Stages and deadlines

Every stage carries a binding deadline, so a facility always knows where the process stands and who it is waiting on.

  1. 1

    Survey request

The facility completes the mandatory requirements, then submits a survey request together with the application letter, self-assessment results, and other registration documents.

  2. 2

    Institution response

The institution states whether the request is accepted within 2 working days of it arriving.

  3. 3

    Document verification

Completeness of the requirements is checked within 14 working days. If anything is missing, the facility has 5 working days to supply it.

  4. 4

    Agreement and scheduling

The survey date, costs, and terms are set out in a written agreement; the surveyor team is appointed free of any conflict of interest.

  5. 5

    Conducting the survey

Supporting documents for each assessment element are uploaded at least 7 days before the survey. The survey is conducted on site or in hybrid form depending on the facility type.

  6. 6

    Decision and certificate

The surveyor report is submitted within 2 working days, the institution’s recommendation within 3 working days, and the electronic certificate is issued within 14 working days of the survey.

Deadlines follow the Accreditation Survey Technical Guidelines (Director General of Health Services Decree No. HK.02.02/I/3991/2022).

Frequently asked questions

How do the requirements differ between a first clinic accreditation and re-accreditation?

Both require an active business licence, valid STR and SIP for care providers, and validated ASPAK data. The difference is the reporting window for National Quality Indicators and Patient Safety Incidents: the last 3 months for a first survey, the last 12 months for re-accreditation.

How many assessment elements does clinic accreditation have?

The clinic accreditation standard comprises 3 chapters, 22 standards, and 104 assessment elements: Clinic Governance, Quality Improvement and Patient Safety, and Delivery of Individual Health Services.

How long does a clinic accreditation survey take?

A clinic survey runs for 2 days with 2 surveyors using the hybrid method: 1 day online for document review and 1 day on site for field tracing.

Is a clinic without laboratory services still assessed on the laboratory chapter?

Assessment elements that are not relevant to the services actually provided are declared not applicable and excluded from the score calculation.
